Covenant Spine & Neurology provides our patients with downloadable office forms for your convenience. We recommend that you download, print, and fill out the necessary forms prior to your appointment to save time.
APPOINTMENTS / CANCELLATIONS
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment, we require 24 hours notice. In such a case, please call our office and arrange to reschedule your appointment with one of our patient care coordinators.
ACCEPTED INSURANCE PLANS
Our practice accepts most major insurance plans and most of the smaller insurance plans. If we do not participate with your insurance and your plan allows out of network benefits, our office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.
